Abstract
Berdasarkan data Rekam Medis RSUD Dr. Rasidin Padang (2024), penderita diabetes melitus pada bulan Januari-Juni berjumlah 335 orang. Ulkus diabetikum adalah komplikasi kronik diabetes berupa luka terbuka pada permukaan kulit yang dapat disertai dengan adanya kematian jaringan. Tujuan penulisan karya ilmiah akhir ners ini untuk mengaplikasikan asuhan keperawatan pada Ny. S dengan penerapan modern dressing menggunakan hydrogel untuk mengatasi gangguan integritas kulit post operasi debridement ulkus diabetikum di ruangan Zaitun RSUD Dr. Rasidin Padang.
Hasil pengkajian didapatkan bahwa Ny. S mengalami post operasi debridement ulkus diabetikum dengan luas luka ± 3 cm x 2 cm, kedalaman luka ± 1 cm, warna luka kemerahan, terdapat pus dan berbau khas. Dari kasus tersebut, maka dapat dirumuskan diagnosa keperawatan prioritas, yaitu gangguan integritas kulit. Salah satu intervensi non farmakologis yang dapat dilakukan untuk mengatasi gangguan integritas kulit post operasi debridement ulkus diabetikum, yaitu penerapan modern dressing menggunakan hydrogel. Modern dressing menggunakan hydrogel adalah suatu cara terbaik mendebridement jaringan nekrotik untuk mempercepat granulasi.
Implementasi dilakukan dari tanggal 24-26 Juli 2024 dan evaluasi yang hasil luka yang sudah mulai membaik walaupun belum sembuh sepenuhnya. Dimana luka post operasi debridement ulkus diabetikum pada kedua telapak kaki dan di sela jari kelingking kaki bagian kiri pasien tampak mulai membaik ditandai dengan luka tampak kering dan bersih.
Berdasarkan hasil penulisan karya ilmiah akhir ners ini dapat disimpulkan bahwa penerapan modern dressing menggunakan hydrogel efektif untuk mengatasi gangguan integritas kulit post operasi debridement ulkus diabetikum. Diharapkan bagi perawat di ruangan Zaitun RSUD Dr. Rasidin Padang dapat memberikan secara rutin modern dressing menggunakan hydrogel ini untuk mengatasi gangguan integritas kulit post operasi debridement ulkus diabetikum.
Based on data from the Medical Record of Dr. Rasidin Padang Hospital (2024), there were 335 people with diabetes mellitus in January-June. Diabetic ulcers are a chronic complication of diabetes in the form of open wounds on the skin surface that can be accompanied by tissue death. The purpose of writing this final scientific work is to apply nursing care to Mrs. S with the application of modern dressings using hydrogel to overcome impaired skin integrity after diabetic ulcer debridement surgery in the Olive room at Dr. Rasidin Hospital Padang.
The results of the assessment found that Mrs. S experienced postoperative debridement of diabetic ulcers with a wound area of ± 3 cm x 2 cm, wound depth ± 1 cm, reddish wound color, pus and characteristic odor. From these cases, priority nursing diagnoses can be formulated, namely impaired skin integrity. One of the non-pharmacological interventions that can be done to overcome impaired skin integrity after diabetic ulcer debridement surgery is the application of modern dressings using hydrogel. Modern dressing using hydrogel is an excellent way of debridement of necrotic tissue to accelerate granulation.
Implementation was carried out from July 24-26, 2024 and evaluation of the results of wounds that have begun to improve even though they have not fully healed. Where the postoperative wound debridement of diabetic ulcers on both soles of the foot and between the little fingers of the patient's left foot looks starting to improve marked by the wound looking dry and clean.
Based on the results of writing this final scientific work, it can be concluded that the application of modern dressings using hydrogel is effective for overcoming impaired skin integrity after diabetic ulcer debridement surgery. It is hoped that nurses in the Olive room of Dr. Rasidin Padang Hospital can provide routine modern dressings using this hydrogel to overcome impaired skin integrity after diabetic ulcer debridement surgery.
